Referral via Email

AWP has introduced an email referral form for all urgent (72 hour) and routine referrals (4 weeks) to PCLS/Triage. The form should be available as an EMIS template.

After 13 July 2018 faxes will no longer be accepted and referrers should be referring for 72 hour and 4 week responses via the email referral form.

In North Somerset & South Gloucestershire, GPs should phone through any 72 hour referrals on the following numbers:

North Somerset Tel: 01934 836406                        South Glos: Tel: 0117 3787960

Out of office hours (8am - 5.30/6pm) calls will be diverted to the relevant on call team. A referral form may be required following the phone conversation.

Upon completion, the email referral should be emailed to the relevant locality PCLS/Triage Team for action:

Bristol: awp.BMHTriage@nhs.net

North Somerset: awp.nsompcl@nhs.net

South Gloucestershire: awp.referralsonlysglospcls@nhs.net

You should have received a copy of the email referral form and uploaded this into your EMIS system. If you have not yet done so and need a copy of the email referral form, guidance or further assistance during the transition to paperless referrals, please contact:

Bristol: abraham.chipuriro@nhs.net

North Somerset: kate.ingarfield@nhs.net

South Gloucestershire: cindey.kurylo@nhs.net
