District Nursing and Community Matrons

Must be registered with a North Somerset GP and aged over 18 years.

Community Nursing

Provision of Nursing care for housebound patients

Community Matrons

Provision of care for people with complex care needs and/or chronic long term condition(s), including frailty.

Community Rehabilitation

Must be registered with a North Somerset GP and aged over 18 years

Provision of rehabilitation, support and advice along distinct pathways:

Intensive therapy team - home based therapeutic care to people in their usual place of residence

 Community Rehabilitation Team (CRT) – incorporating community physiotherapy/occupational therapy, the Community Neuro Service and Falls (see separate referral route)

Referrals

Monday - Friday, excluding BHs

08.30 -17.00 hr

Discharge to Assess

Must be registered with a North Somerset GP and aged over 18 years.

Pathway 1 (in usual place of residence) and Pathway 2 (bed based – at North Somerset Community Hospital)
