Community Eye Service

The Community Eye Service provides accessible community based services, ensuring that patients are given quick access to advice, diagnosis and treatment for a range of non-sight threatening, non-surgical conditions that would have been traditionally referred to the hospital eye service. The service is overseen by lead ophthalmologist, Mr Simon Hardman-Lea and clinics are currently provided in Thornbury, Fishponds and Henleaze.

To reduce delays and maximise the patient experience, Evolutio can accept referrals directly from Optometrists. Optometrists will be able to send referrals securely to the Community Eye Service via eRefer (Evolutio’s electronic, NHS compliant  IT platform), NHS mail or fax.    Optometrists will use the same inclusion/exclusion criteria and any inappropriate referrals will be returned to the Optometrist for onward referral to the hospital eye service via the patient’s GP. Please note that cataract is not included in the specification for the community eye service and would GPs please refer directly to secondary care via eReferral.
