Service Overview - Bristol
Please note that this service is only available to patients registered with a GP in the Bristol area
The Bristol Community Health Community Respiratory Service is made up of health care professionals who specialise in providing services for people with chronic respiratory diseases. These services include; preventing unnecessary admission to hospital, supporting early discharge from hospital, home oxygen assessment review and pulmonary rehabilitation. Service Overview - South Gloucestershire
The service involves four elements:
an oxygen assessment and review service for patients needing oxygen therapy
a prevention service to avoid you needing to go into hospital
a hospital at home service following a hospital discharge
an exercise and education programme (pulmonary rehabilitation)
Patients may receive care under all four elements or just one or two depending upon your individual needs. We may also liaise and refer patients on to other services if required.

Service overview - North Somerset
Home oxygen assessments in North Somerset are undertaken by the Home Oxygen Team at NSCP.
The Home Oxygen Service referral form is available as an EMIS template and can be sent via EMIS Managed Referrals or emailed to firstname.lastname@example.org Alternatively, use the referral form in the link above to make a referral.
