Rapid Response - Bristol Community Health
The Bristol Community Health Rapid Response Service (part of the Urgent Care Service) offer care for those who are suffering from acute episodes of illness. They will visit patients in their own home or place of residence for up to 7 days.
Rapid Response aims to facilitate and provide patient care 24 hours a day, 7 days a week, reducing the need for hospital admission.

Referrals are appropriate for all patients over 16 years of age, who are physiologically stable where out of hospital care would be possible.
Referrals should be sent to the Urgent Care Single Point of Access (SPA) - 0117 903 0202. This telephone line is open 24 hours a day, 7 days a week.
GP's can access the NSCP Clinical Hub using one telephone number, which is 01275 888400, 24 hours a day, 7 days a week
The NSCP Clinical Hub is open:
- Monday - Sunday: 8.00am to 10pm
Outside of these hours, all telephone calls will be diverted to a clinician in Rapid Response.

- Safehaven beds
- Rapid Response
- IV Service
The NSCP Clinical Hub work closely with Rapid Response and the Ambulatory Care Centre at Weston General Hospital.
The Community Rapid Response team provides 24-hour intensive nursing assessment and support. This includes short-term care packages to patients with acute health needs, in their own home for up to 14 days.
